Refund Request Form

If you feel you are entitled to a refund of your tax payment, you may request a refund. Your request will be researched and verified and if it is determined that a refund should be issued, you will receive a refund application form. Proof of payment will be required to process all claims. Refunds will take between 6 and 8 weeks to process. For further questions or concerns, please call 216-443-7400.
